Free Mom & Grandma Questionnaires for Mother’s Day
Celebrate Mother’s Day with these free printable questionnaires for mom, mum, or grandma. These keepsakes are so fun for kids to fill out and make a thoughtful gift!
Mother’s Day always creeps up on me and then I’m rushing to find something to give my mom and mother-in-law (or something for my son to give them). You can never go wrong with keepsake gifts, though!
To make your life a little easier, I’ve got a free printable Mother’s Day questionnaire for you with three different versions: one for mom, mum (if you’re outside the US), and grandma. There are also different designs for each so you can pick your favorite to print.
These free printables are a fun activity for young kids as well as older kids. The completed questionnaire makes the perfect gift for Mother’s Day or can even be part of a birthday gift (the wording is neutral so you could really fill this out at any time of year).
The questionnaire includes 13 different questions to complete and I think you’ll love hearing your kids’ answers–they say the funniest things sometimes, so you’ll probably get a good laugh with some of them!

Free printable Mother’s Day questionnaire
There are three different designs for each mother figure:
- a botanical floral design, which is my favorite (pictured below)
- a simple pink daisy border
- and a blank black and white version (perfect if you don’t have a color printer, if you want to print on a colored paper, or for customizing after it’s printed)

The questions are all the same for each person. It asks what mom or grandma’s name is (this could also be what your child calls them), her age (or how old your child thinks they are…this can be entertaining), some of her favorite things like favorite color and favorite food (also fun to see what they choose), and then more sentimental questions like why your child loves them. These fun questions are sure to put a big smile on mom or grandma’s face!
If you’re a preschool or elementary teacher, you’re welcome to use these printable pages too! They make a great addition to any Mother’s Day craft you might have planned for them and can even be a fun way to practice writing skills.
How to use these free Mother’s Day printables

Pick which design you want and download your printable questionnaire below. Print at 100% scale and choose borderless printing (especially important for the floral border design since it goes right up to the edge and you don’t want anything to get cut off). You can print on regular printer paper or white cardstock if you want something a little sturdier. If printing the plain black and white version, you could also print on colored paper.
Once printed, have your child finish the questionnaire prompts by writing in their answers (you may have to write in the answers for younger children). At the bottom, there is a spot to write their name (where it says Love:) and their age and the date the questionnaire was filled out. You and the recipient will both love looking back on your child’s answers when they were this age (even if it is slightly offensive how old they think mom and grandma are 😜)!
The blank black and white version is great for personalizing and could easily be turned into a Mother’s Day craft by making it a coloring page and drawing your own border or doodling around the outside. Or, cut it out and mount it on a piece of patterned paper. You could even put the questionnaire in a picture frame if you want.
Once done, all that’s left to do is give this heartfelt gift to the mother figure in your life! And I know she will love reading the sweet answers about her!
